Failure Mode and Effects Analysis of Plastics Injection Molding Machine Using Fuzzy TOPSIS Method Based on Entropy Weight

Deskripsi/Abstrak

The aim of this paper is to identify and prioritize potensial failure modes of Plastic Injection Molding Machine by using a modified Failure Mode and Effects Analysis (FMEA). The modified FMEA is developed to overcome the limitations and to improve the performance of the conventional FMEA. A modified FMEA combining Entropy weight with fuzzy logic and Technique for Order Preference by Similarity to Ideal Solution (Fuzzy TOPSIS) methods, is proposed in this paper. The Entropy weight is applied to determine the objective weights of the three risk factors of failure modes : severity, occurrence and detection. Fuzzy TOPSIS is used to rank the risk of failure modes and the method is applied because of its ability to deal with imprecision and vagueness in the evaluation process. The study has identified nine failures of an injection molding machine which is used for production in a plastics manufacturing company. The findings showed that the risk factor occurrence has the highest objective weight with weight of 0.384 followed by severity with weight of 0.315 and detection with weight of 0.300. The result also showed that the damage of the hopper seal has the highest priority risk among the nine failures identified.
